APPLICATION FOR INTERNATIONAL MANUFACTURER MEMBERSHIP

International Manufacturer membership in APPMA is open to pet product manufacturers or importers who do not have a manufacturing facility or a warehouse in the United States. If you do have either facility in the U.S., please complete the Application for Domestic Manufacturer Membership.

The undersigned hereby makes application for International Manufacturer membership in American Pet Products Manufacturers Association, Inc. (APPMA) and, if accepted for membership by the Board of Directors, agrees to abide by all the Rules, Regulations and By-Laws of said Association.

Directions: Please complete this form in its entirety, by typing your response or, where appropriate, indicating "Not Applicable." Incomplete information or documentation will delay processing and consideration of this application. The information contained in this application will remain confidential except to the extent necessary for consideration of membership and/or provision of member services. The address and all information on the invoice must match the bill of lading/shipping documentation.

6. Pet Product Sales As you evaluate your dues category, be sure to check the appropriate level based on your company's gross annual world-wide sales of pet and pet-related products and veterinary care products available for retail sale. Accurate dues represent your company's total world-wide sales of these products in the prior twelve months. To ensure accuracy, have your CEO, CFO, certified public accountant or other accounting officer verify the appropriate dues category for your company. Accurate reporting of member dues is essential to the vibrancy of the Association. APPMA reserves the right to conduct random dues level audits at any time. Indicate which category of total world-wide pet product sales is applicable and enclose the stated dues amount with this application. I understand that should this information be incorrect, this application will be rejected. I further understand that if the application is accepted and later discovered to be incorrect, my company's membership will be terminated.
8. Membership applications withdrawn before presentation to APPMA's Board of Directors will be assessed a $75.00 processing fee. No credit or refund will be issued for the withdrawal of a membership application after the Board's approval, regardless of reason.

Please be sure to include the following enclosures in English or submitted with English translations:

• Application completed in English and signed by an officer of the applicant. (Items left blank or unclear constitute an incomplete application which cannot be considered.)
• Copy of the product brand name, registered trademark or trade name with the proper documentation (as indicated in item #3 of this application).
• Copy of one (1) invoice with a matching bill of lading indicating sales of applicant's pet products (as indicated in item #4 of this application).
• Initiation fee (1,000 USD) plus first year’s dues based on your annual world wide sales of pet products (as indicated in item #6 of this application).  Payment must be made in U.S. dollars either by check, money order, credit card or wire transfer.   For wire transfer information, please contact: Miranda@appma.org.

Please mail the completed application, the product name or mark information, one (1) invoice originating from your manufacturing facility or warehouse with a matching bill of lading and the initiation fee and dues payment in U.S. dollars to:
